Please fax your request with payment to the AAHKS office at (847) 698-0704 or mail to 6300 N. River Road, Suite 615, Rosemont, IL, 60018-4237. No orders will be processed without payment (Check, VISA, MasterCard or American Express only). No phone orders accepted. Please allow one to two weeks for delivery.
