AAHKS will utilize the Orthopaedic Surgeons Disclosure Program located in the member's area of the AAOS website. AAOS members should log in to the disclosure program at: www.aaos.org/disclosure to add or update their disclosure information before the April 30, 2008 deadline. If you do not have an AAOS ID number, please print and complete the AAHKS disclosure form and submit no later than April 30, 2008.
